Aetna covers serologic testing for celiac disease (IgA anti-TTG, IgA/IgG DGP, and IgA EMA) for initial evaluation of symptomatic patients, monitoring response to a gluten‑free diet, and screening first‑degree relatives and persons with type 1 diabetes. Aetna considers HLA genotyping for screening in type 1 diabetes, IgA/IgG anti-gliadin and antireticulin assays, serology as a substitute for biopsy to assess mucosal damage, and a long list of other tests (e.g., D-xylose, intestinal permeability, stool/saliva tests, gene profiling) experimental/investigational and not covered; HLA‑DQ2/DQ8 testing is not recommended routinely and is used only to rule out disease in selected situations.
