ECoG (CPT 92584) is covered when used to evaluate episodic dizziness (vertigo/imbalance) or tinnitus to rule out endolymphatic hydrops (Meniere’s disease) or perilymphatic fistula, and is covered when performed with ABR for profound hearing loss (pure tone average ≥90 dB HL at 500, 1,000, 2,000 and 4,000 Hz); covered ICD‑10 codes include H81.01–H81.09 when selection criteria are met. ECoG is excluded (experimental/investigational) for routine hearing screening, for indications not explicitly listed in the policy (eg, use during superior semicircular canal dehiscence repair or canal resurfacing), for intraoperative evaluation during cochlear implantation, and for measurement of perilymphatic pressure for Meniere’s disease or idiopathic sudden sensorineural hearing loss.
