Ankle Orthoses, Ankle-Foot Orthoses (AFOs), and Knee-Ankle-Foot Orthoses (KAFOs)

Ankle air‑stirrups, reusable elastic ankle sleeves, lace‑up ankle braces, and orthopedic ankle cast‑braces are covered as medically necessary for acute ankle injuries (fractures/sprains), with lace‑up braces also covered for chronic instability and re‑injury prevention; air‑stirrups, elastic sleeves, Orthoplast stirrups and rigid/semi‑rigid casts are considered experimental/investigational when used for chronic instability or to prevent re‑injury (and certain cast uses are investigational as specified). Coverage requires a prescription by a qualified prescriber, documentation that the orthosis will significantly improve/restore MRADLs and permit ADLs on physical exam, provision within six months by a licensed/certified orthotic/prosthetic provider (or under direct supervision), and the orthotist/prosthetist must be in good standing with ABC/BOC or state licensure.
