Aetna covers home/telespirometry (CPT 94014, 94015, 94016; HCPCS E0487) only for lung transplant recipients for home monitoring of lung function (ICD‑10 T86.810–T86.819, Z94.2) and considers it experimental/investigational and not covered for other indications (e.g., asthma, cystic fibrosis, idiopathic pulmonary fibrosis, interstitial lung disease, emphysema and other chronic pulmonary diseases). Coverage requires the service meet the policy’s selection criteria (i.e., use specifically for post‑transplant monitoring/complications or transplant status) and the listed codes may be billed only when those criteria are met; home spirometry is distinct from incentive spirometry and peak flow meters.
