Laser Treatment for Psoriasis and Other Selected Skin Conditions

Aetna covers excimer and pulsed‑dye laser for mild‑to‑moderate localized plaque psoriasis (≤10% BSA) only after ≥3 months of topical therapy with inadequate response and trial of at least three agents from anthralin, topical corticosteroids, keratolytics, retinoids, tar, or vitamin D derivatives; it also covers laser hair removal for recurrent pilonidal cyst, laser excision/depilation for specified acne keloidalis nuchae indications, and laser treatment for penile intraepithelial neoplasia (specified ICD‑10 codes). Coverage is limited to ≤13 treatments per course and ≤3 courses/year, additional courses aren’t medically necessary without objective response (e.g., PASI), and use for non‑plaque psoriasis, combination pulsed‑dye+UVB, and many other dermatologic conditions is considered experimental/investigational and excluded.
