Policy Summary
Aetna covers infrared coagulation for anal dysplasia and for painful or persistently bleeding grade I–II internal hemorrhoids, and allows infrared heat modalities in physical therapy; CPT 46930 and ICD‑10 K62.82 are covered when the plan's selection criteria are met. Low‑level infrared light devices (eg, Anodyne Therapy System) are considered experimental/investigational and not covered for acne, lumbar/thoracic back pain, Bell's palsy, bone regeneration, or brain disorders (including dementia).
