Aetna covers FDA‑cleared air‑conduction hearing aids (including FDA‑cleared OTC devices with a prescription) and selected implantable/semi‑implantable hearing prostheses when medical necessity is met — hearing thresholds ≥40 dB at 500, 1,000, 2,000, 3,000 or 4,000 Hz, or ≥26 dB at three of those frequencies, or speech recognition <94%; implantable/semi‑implantable devices are limited to moderate‑to‑severe sensorineural loss in patients who cannot tolerate an ear mold (e.g., auricular atresia or severe chronic otitis externa). Most Aetna plans exclude hearing aids so coverage additionally requires the member’s benefit plan to allow hearing aids, a prescription from a qualified provider, and FDA clearance; use for balance, depression, cognitive decline or other non‑approved indications is considered experimental/investigational and not covered.
