Aetna covers plasma BNP testing (CPT 83880) to differentiate heart-failure versus pulmonary causes of dyspnea, to assess prognosis or severity in chronic heart failure, and on admission for acutely decompensated heart failure, but considers serial BNP/NT‑proBNP measurements and use as biomarkers for cerebral small vessel disease, hypertensive disorders of pregnancy, subclinical brain damage, or Friedreich ataxia investigational and not covered. Coverage is contingent on meeting the plan’s selection criteria and covered ICD‑10 diagnoses (e.g., I50.x, J40–J47.x, selected R06 codes); tests performed outside those criteria are not covered.
