Aetna covers specialized vacuum systems (eVAC, Harmony VASS, LimbLogic VS) for lower‑limb prostheses when the standard prosthetic medical‑necessity criteria are met and the patient has excessive socket pistoning not correctable by suspension adjustments, excessive residual‑limb hyperemia, or excessive skin hyperhidrosis; use for multiple falls in transfemoral amputees and magnetic panel enlargement of transtibial sockets are considered experimental/investigational and not covered. Key requirements: devices must be prescribed by a qualified clinician and provided within six months of prescription by a licensed/certified orthotist/prosthetist (or appropriate supervised provider) who documents that the prosthesis will restore MRADLs and meets ABC/BOC or state licensure standards.
