Osteochondral autografts (OATS/mosaicplasty) are covered for symptomatic focal full‑thickness (Outerbridge III/IV) unipolar defects on the weight‑bearing femoral condyles or trochlea in skeletally mature, non–total knee replacement candidates (documented growth plate closure, generally ≥15 years and ≲55 years) who have failed appropriate non‑surgical therapy, with surrounding cartilage ≤Outerbridge II and a stable, aligned knee with intact meniscus and normal joint space (corrective procedures may be required). Use of OATS/mosaicplasty for other joints (ankle, elbow, hip, patella, shoulder) is experimental/investigational and not covered.
