Aetna covers autotransfusers (intra‑operative/wound collection, hemodilution/cell washing, and post‑operative chest‑tube reinfusion usually within 2 hours) as medically necessary for procedures with potential major blood loss (e.g., cardiopulmonary bypass/high‑risk cardiac, abdominal aortic, ectopic pregnancy, hysterectomy, organ transplant, major orthopedics, vascular grafts, emergency hemorrhage). They are experimental/investigational for other indications, not medically necessary if expected blood loss is <2 units, contraindicated for blood contaminated by bacteria/fecal material or malignant cells, and CPT codes 86890/86891 require meeting the policy’s selection criteria.
