Aetna covers therapeutic phlebotomy for erythrocytosis of undetermined etiology when hematocrit is ≥55%, for hemochromatosis (including hereditary), for individuals on testosterone therapy with hematocrit ≥54% (or hemoglobin ≥180 g/L), for nonalcoholic fatty liver disease with hyperferritinemia, and for non‑hereditary iron overload with elevated hepatic iron concentration. Aetna considers therapeutic phlebotomy experimental/investigational (and not covered) for acute gouty arthritis, chronic hepatitis C as interferon adjunctive therapy, chronic urticaria, the common cold, and hemoglobin SC disease.
