Aetna covers metal‑on‑metal hip resurfacing as a medically necessary alternative to total hip arthroplasty for physically active adults under 65 with advanced hip disease (pain/functional disability from osteoarthritis, avascular necrosis, or post‑traumatic arthritis), limited ROM/antalgic gait, imaging evidence of severe OA (≥2 radiographic/MRI signs) or early femoral‑head osteonecrosis with <50% involvement, and normal proximal femoral bone geometry and bone quality. It is experimental/investigational for all other indications and is not medically necessary for patients with active joint or systemic infection, active skin infection or open wound at the surgical site, metal allergy (e.g., cobalt/chromium/alumina), or inactive/older individuals unlikely to require revision of a traditional THR.
