Policy Summary
Aetna covers one ocular photoscreening (CPT 99174 or 99177 when selection criteria are met) as medically necessary for all children at age 3 and for children 4–5 only if they cannot cooperate with routine acuity testing, with positive/abnormal screens requiring referral for a comprehensive eye exam. Retinal birefringence scanning (CPT 0469T) and photoscreening for strabismus-related indications are considered experimental/not covered.
