Aetna covers hydrogen breath testing (CPT 91065) only for suspected lactose intolerance after a 2‑week trial of a lactose‑free diet with persistent symptoms, and covers fractional exhaled nitric oxide testing (CPT 95012) for asthma evaluation and for monitoring response to long‑term controller therapy in patients aged ≥5 years, with coverage contingent on meeting the policy’s selection criteria. Exhaled breath temperature, gastric emptying breath tests, breath tests for diabetes or gastric cancer, hydrogen breath testing for other indications (e.g., SIBO, IBS, gastroparesis), and exhaled breath condensate pH for listed conditions are considered experimental/investigational and not covered.
