Aetna considers Gardasil 9 medically necessary as a preventive HPV vaccine for persons aged 9–45 years (including to continue/complete a 3‑dose series when the previously given product is unknown), with dosing per manufacturer/ACIP guidance (three‑dose series; ACIP allows a 2‑dose schedule if started at ages 9–14). It is not necessary for those who have already completed a three‑dose Gardasil or Cervarix series, is investigational for other indications (e.g., treatment of active warts, post‑treatment adjunctive use, juvenile recurrent respiratory papillomatosis, certain anal lesions), CPT codes 90649/90650 are not covered, and members should verify preventive coverage in their benefit plan.
