Nerve Fiber Density Measurement

Aetna covers intra-epidermal nerve fiber density (IENFD) by skin biopsy for diagnosis of small‑fiber neuropathy only when ALL criteria are met: painful sensory neuropathy, no history of disorders that predispose to painful neuropathy (e.g., diabetes, toxic, HIV, celiac, inherited), no clinical signs of large‑fiber neuropathy, and normal EMG/nerve conduction studies (ICD‑10 G60.8 if criteria met). Measurement of IENFD is experimental/investigational and not covered for monitoring disease progression or treatment response, nor for preclinical hypothyroid neuropathy, endometriosis diagnosis, evaluation of hereditary or iatrogenic TTR amyloidosis, Ehlers‑Danlos syndromes, or other unestablished indications.
