Policy Summary
Aetna considers the Menaflex (collagen meniscal implant/scaffold) experimental and investigational and excludes coverage (HCPCS G0428) for repair/reinforcement of the medial meniscus and all other listed indications. Coverage is denied for diagnoses including osteoarthritis of the knee (M17.0–M17.9), internal derangement codes (M22.2x1–M23.92; Q68.6) and other specified knee disorders (M25.161–M25.169; M25.861–M25.869) due to insufficient evidence of effectiveness.
