Venous stenting is covered for patients with medically refractory idiopathic intracranial hypertension (IIH), provided they meet specific criteria, including documented vision loss, neurologic examination findings, and neuroimaging results that exclude other conditions. Key requirements include intracranial pressure elevation during lumbar puncture and evidence of stenosis via MRV, with prior authorization waived if a pressure gradient greater than 8 mmHg is documented. The procedure is not covered for patients with secondary causes of intracranial hypertension or if specific limitations, such as systolic blood pressure exceeding 150 mmHg, are met.
