Concert Genetic Testing: Preimplantation Genetic Testing
AMBETTER-CG-Preimplantation-Testing-2025.1
This policy covers preimplantation genetic testing (PGT) performed with in vitro fertilization—including PGT‑M for known parental single‑gene mutations, PGT‑SR for parental structural chromosomal rearrangements, and PGT‑A aneuploidy screening. Coverage is limited to specified indications (e.g., known parental carriers or one parent with an autosomal dominant/X‑linked mutation or a parental chromosomal rearrangement), requires genetic counseling and offering diagnostic confirmation (CVS/amniocentesis) when applicable, and explicitly considers PGT‑A and other uses outside these indications investigational/not medically necessary with routine universal blastocyst biopsy unsupported.
