Deep transcranial magnetic stimulation (dTMS) using an FDA‑cleared H‑7 coil targeting the anterior cingulate cortex/medial prefrontal cortex is covered for treatment of obsessive‑compulsive disorder (OCD). Coverage is limited to adults (≥18) with primary DSM‑diagnosed OCD who, during the current episode, have failed adequate CBT/ERP plus multiple pharmacologic trials (including ≥4 trials across ≥2 classes or documented intolerance/incompatibility), meet Y‑BOCS clinical thresholds, receive treatment under appropriate psychiatric supervision (standard 5×/week for 6 weeks, up to 36 sessions), and have no disqualifying comorbid psychiatric disorders, implanted ferromagnetic/electronic devices, recent substance‑use relapse, or other listed contraindications; retreatment and maintenance have additional strict criteria and case‑by‑case review.
