This policy covers excimer (308 nm) laser–based targeted phototherapy and related localized UV phototherapy (e.g., narrowband UVB, broadband UVB) for treatment of specified inflammatory and depigmenting skin conditions. It is considered medically necessary for adults with conditions such as localized plaque psoriasis (including <10% BSA and individual lesions), vitiligo, atopic dermatitis, and cutaneous T‑cell lymphoma when topical therapies have failed, but pediatric use has limited evidence, treatment is typically delivered in‑office 2–3 times weekly, and evidence is insufficient for photosensitivity disorders or indications not specifically listed.
