This policy covers posterior tibial nerve stimulation (PTNS) — 30‑minute percutaneous tibial nerve stimulation sessions weekly for an initial 12‑week course — for adults with overactive bladder (OAB) and associated symptoms (urgency, frequency, urge incontinence) who have failed or are intolerant of conservative management and pharmacotherapy and who demonstrate clinical response to the initial 12 treatments. Coverage is limited to third‑line use after multidisciplinary review and requires patient willingness/ability to attend frequent office visits for ongoing maintenance (typically individualized tapering to ~monthly sessions); evidence is primarily short‑ to medium‑term with insufficient data beyond 12 months, and implantable tibial nerve neuromodulation devices are investigational and not covered.
