This policy covers planned home births when mandated by law or benefits and when provided as medically necessary by a participating, credentialed Plan provider (e.g., AMCB‑certified midwife or obstetrician) for appropriately selected low‑risk patients — singleton, cephalic pregnancies with no prior cesarean, no significant maternal disease, and spontaneous labor between 37 0/7 and 41 6/7 weeks (outpatient induction 39 0/7–41 6/7 weeks). Coverage requires a written emergency‑care/transport plan able to reach a hospital within 15 minutes, two planned providers (including an NRP‑certified infant caregiver), admitting‑privileged supervising obstetrician/ABOG‑qualified backup, and services that do not meet these staffing, supervision, admitting‑privilege, transport or low‑risk selection requirements are excluded.
