This policy covers surgical debridement for removal of large necrotic or infected tissue and for management of chronic, partial‑ or full‑thickness non‑healing wounds, and it also addresses low‑frequency ultrasound and noncontact normothermic wound therapy (NNWT/MIST). Major limitations: low‑frequency ultrasound and NNWT are not supported by current evidence and are not covered—available studies are small, underpowered, and inconclusive, so routine use (including ultrasonic debridement instead of surgical debridement for venous leg ulcers) is not endorsed pending larger randomized trials.
