Key populations include patients with neuromuscular disease, severe thoracic cage disorders, obesity hypoventilation syndrome (BMI ≥30) and select severe COPD cases, but coverage requires stability with qualifying testing (e.g., awake ABG with PaCO2 ≥45 mmHg or sleep oximetry criteria), is initially authorized for a 3‑month period, excludes use intended for RAD/CPAP, and continued coverage requires ≥4 hours average use/24h and documented clinical benefit.
