ADMIN.00002 Preventive Health Guidelines

Policy addresses: Preventive health services guided by recommendations from Anthem-referenced organizations (focused on primary and secondary prevention). Covered when the service is recommended by these organizations; updates to those recommendations are considered medically necessary as of their stated effective date, but any topic-specific Anthem medical policy or clinical UM guideline takes precedence. Not covered under this policy when a service is not supported by the referenced recommendations, is outside preventive intent, or is governed by a more specific Anthem policy or guideline.
