CG-LAB-11 Vitamin D Testing

This policy addresses laboratory testing of vitamin D levels. Testing is medically necessary for individuals at high risk of deficiency or intoxication—such as those with aggressive sun protection limiting sun exposure, adolescents with amenorrhea, chronic kidney disease, or chronic use of select medications (e.g., anti-seizure drugs, glucocorticoids, antiretrovirals, antifungals)—for diagnosis and ongoing management. It is not medically necessary for all other indications, including routine screening of asymptomatic, average-risk individuals who do not meet high-risk criteria.
