CG-LAB-16 Serum Amylase Testing

This policy addresses serum amylase testing (CPT 82150). Anthem considers serum amylase testing not medically necessary—and therefore not covered—for all indications, including diagnosis or management of acute and chronic pancreatitis. There are no covered indications or exceptions under this policy.
