CG-LAB-27 Human Chorionic Gonadotropin Testing

This policy addresses laboratory testing for human chorionic gonadotropin (hCG), including urine and blood assays. Coverage is medically necessary for urine hCG to confirm pregnancy, and for blood hCG to confirm pregnancy, to screen pregnant individuals for fetal abnormalities as part of prenatal screening, and for evaluation of hypertensive disorders of pregnancy. Urine or blood hCG testing is not covered for any other indications.
