CG-LAB-28 Prostate Specific Antigen Testing

This policy addresses prostate-specific antigen (PSA) blood testing. PSA testing is medically necessary for prostate cancer screening; evaluation of signs or symptoms suggestive of prostate cancer; in individuals with previously elevated or rising PSA levels; or in those with a current or past history of prostate cancer, and is not medically necessary for any other indications.
