This policy addresses preoperative testing before low-risk invasive procedures and non-cardiac surgeries, including CBC/coagulation/metabolic panels, urinalysis, chest X-ray, resting ECG/echocardiogram, and pre-procedure consultations. Testing is covered when selectively ordered based on clinical indications from history/physical, comorbidities, or procedure-specific risk; it is not covered as routine screening in asymptomatic patients aged 16–65 with ASA I–II and no risk factors, or when the same tests were already performed within 30 days. Coverage emphasizes a selective, not routine, approach consistent with ASA guidance.
