CG-MED-66 Cryopreservation of Oocytes or Ovarian Tissue

This policy addresses cryopreservation of mature oocytes and ovarian tissue. Mature oocyte cryopreservation is medically necessary when the individual is post‑pubertal, an appropriate candidate based on ovarian reserve/likelihood of success (e.g., age ≤45), and anticipates infertility from medical or surgical treatment (chemotherapy, radiation, other gonadotoxic therapy, or bilateral oophorectomy); it is not covered for infertility due to elective sterilization or normal reproductive aging. Oocyte cryopreservation is not medically necessary when these criteria are not met, and ovarian tissue cryopreservation is not medically necessary.
