ANTHEM-CG-MED-92
CG-MED-92 addresses foot care services, including cutting/removal of corns and calluses and trimming/cutting/clipping/debriding of nails. These services are medically necessary when the individual has a systemic condition causing circulatory insufficiency or desensitization of the lower extremity, such as diabetes mellitus, peripheral vascular disease, or peripheral neuropathy. They are not covered as routine foot care when these criteria are not met (including mycotic nail care), and cleaning/soaking of the feet (e.g., whirlpool or hydrotherapy) is not medically necessary.
