CG-MED-97 Biofeedback and Neurofeedback

Policy CG-MED-97 addresses biofeedback therapy and neurofeedback (EEG biofeedback). Biofeedback is covered as medically necessary when supervised by a physician or licensed practitioner and used to treat cancer pain, chronic back pain as part of a rehabilitation program, chronic constipation, fecal incontinence, levator ani (anorectal pain) syndrome, migraine or tension headaches, or urinary incontinence. Biofeedback is not covered when these criteria are not met or for any other indications, and neurofeedback is considered not medically necessary for all indications (e.g., asthma, ADHD, autism spectrum disorders).
