CG-SURG-05 Maze Procedure

This policy addresses the surgical Maze (Cox-Maze) procedure for treating atrial fibrillation (AF) or atrial flutter. It is covered as medically necessary for patients with drug‑resistant AF or flutter, and for individuals with highly symptomatic AF who require open‑heart surgery for valvular, ischemic, or congenital heart disease. The Maze procedure is not covered for all other indications.
