CG-SURG-128 Presbyopia and Astigmatism-Correcting Intraocular Lenses

This policy addresses presbyopia-correcting (accommodating/multifocal) and astigmatism-correcting (toric) intraocular lenses used in cataract surgery. Anthem deems these lenses not medically necessary for all indications and therefore not covered; claims using HCPCS codes V2787 and V2788 are typically denied. There are no covered indications under this policy; standard monofocal intraocular lenses are not affected and follow usual cataract surgery benefits.
