CG-SURG-25 Injection Treatment for Morton’s Neuroma

This policy addresses injection treatment for Morton’s neuroma using anesthetic, sclerosing (neurolytic), or steroid agents. Coverage is medically necessary only after failure of all conservative measures: padding/orthotics, activity modification, shoe-wear changes (e.g., wider toe box), and medications unless contraindicated; injections are not medically necessary if these criteria are not met.
