Policy Summary
This policy addresses destruction of pre-malignant skin lesions using methods such as laser surgery, electrosurgery, cryosurgery, chemosurgery, or surgical curettage. It is covered as medically necessary when the lesion is pre-malignant (e.g., actinic keratosis, lentigo maligna, squamous cell carcinoma in situ/Bowen’s disease). It is not covered for benign/non–pre-malignant lesions or cosmetic removal (e.g., skin tags, common warts); coverage requires documentation that the lesion is pre-malignant.
