CG-SURG-82 Bone-Anchored and Bone Conduction Hearing Aids

This policy addresses bone-anchored and bone conduction hearing aids, including fully or partially implantable systems and transcutaneously worn devices applied with a headband or adhesive adapter. Coverage is medically necessary for implantable devices in individuals age 5 or older (including as an alternative to air conduction aids) and for transcutaneous headband/adhesive devices at any age, when both audiologic and medical condition criteria are met; for bilateral implants, criteria include conductive or mixed hearing loss with symmetric bone-conduction thresholds. Devices and replacement parts/upgrades are not covered when these criteria are not met.
