DME.00050 Remote Devices for Intermittent Monitoring of Intraocular Pressure

This policy addresses remote devices used for intermittent monitoring of intraocular pressure (IOP), including home or ambulatory IOP monitors. Anthem considers these devices investigational and not medically necessary; therefore, they are not covered for any indication. There are no exceptions or covered indications, regardless of diagnosis or glaucoma type (claims may be submitted under HCPCS E1399 but will be denied).
