This policy addresses brain-computer interface (BCI) rehabilitation devices, including EEG-driven upper extremity powered exercisers (e.g., IpsiHand) used for post-stroke motor recovery. Anthem deems these devices investigational and not medically necessary for any indication; therefore, they are not covered (e.g., HCPCS E0738). No coverage exceptions or patient selection criteria apply, irrespective of FDA de novo classification.
