This policy addresses proprietary algorithm-based laboratory tests that calculate liver fibrosis severity (multi-marker/computational scores). Anthem considers these tests investigational and not medically necessary; they are not covered for any indication, including evaluation or monitoring of hepatic fibrosis due to hepatitis C or other liver diseases. There are no covered circumstances under this policy (e.g., services billed under 81517, 81596, 81599, 84999 are non-covered).
