LAB.00027 Selected Blood, Serum and Cellular Allergy and Toxicity Tests

This policy addresses selected blood, serum, and cellular tests marketed for allergy/toxicity assessment, specifically ALCAT, basophil activation testing (BAT), cytotoxic leukocyte testing, and the HEMOCODE Food Tolerance System. Anthem considers these services investigational/experimental and not medically necessary; therefore, they are not covered for any indication, including evaluation of food allergy or intolerance. This noncoverage applies regardless of diagnosis or setting; established diagnostic methods (such as skin prick testing, allergen‑specific serum IgE, elimination diets, and oral food challenges) are not affected by this policy and may be covered under separate policies.
