LAB.00045 Selected Tests for the Evaluation and Management of Infertility

This policy addresses selected laboratory tests used in infertility evaluation/management: sperm capacitation testing, sperm DNA fragmentation testing, sperm penetration assay, and uterine natural killer (uNK) cell testing. Anthem considers all of these tests investigational and not medically necessary for diagnosing or managing infertility; therefore, they are not covered. No covered indications or exceptions are specified; noncoverage applies across settings, with related codes including CPT/HCPCS 86357, 89329, 89330, 89398 (often used with ICD-10 N97.0, N97.9).
