This policy addresses laboratory testing to detect per- and polyfluoroalkyl substances (PFAS) in human specimens. PFAS testing is considered investigational and not medically necessary; therefore, it is not covered for any indication, including screening, diagnostic evaluation, exposure assessment, or monitoring. Non-coverage applies across all settings and methods (e.g., CPT 82542, 83921, 84999), with no exceptions or medical-necessity criteria.
