Policy ANTHEM-MED.00089 addresses quantitative muscle testing (QMT) devices used to objectively measure muscle strength, including dynamometers and devices such as MedX Lumbar and Cervical Extension Machines for isometric/isokinetic testing. Anthem considers QMT devices investigational and not medically necessary; therefore, they are not covered. This non-coverage applies to all indications and settings (regardless of FDA 510(k) clearance), and related claims (e.g., CPT 97799) are denied as investigational.
