Coverage: Anthem deems intracardiac ischemia monitoring investigational and not medically necessary for all indications; there are no covered circumstances. Limitations: Services/devices billed under HCPCS/CPT codes 93799, C1833, and C9750 are not covered, and no exceptions are specified.
