This policy addresses wearable devices used to manage, monitor, or prevent stress and stress-related conditions, including audio-visual entrainment (AVE) wearables. Anthem considers these devices investigational/experimental and not medically necessary; they are not covered for any indication. This non-coverage applies to all diagnoses and uses (e.g., ICD-10 F43.0, F43.9, R45.7, R45.89, Z56.6, Z73.3) and may be submitted under unlisted codes such as 99199 and E1399.
