This policy addresses epiduroscopy (epidural spinal endoscopy/epidural myeloscopy), which uses a flexible endoscope to visualize the epidural space and may guide interventions such as lysis of adhesions or steroid delivery. Anthem considers epiduroscopy investigational and not medically necessary; therefore, it is not covered. This noncoverage applies to all indications and settings—diagnostic or therapeutic—and includes services reported with unlisted code 64999.
